Case manager III who has extensive experience in and Re-entry program providing formerly incarcerated clients with mental illness, chronic health issues, and substance use disorders services. Which consist of assisting  with  linkages, coordination, and monitoring of services. Providing advocacy with helping to Identify and access resources to meet needs and assist clients in maintaining resources and benefits, while empowering them to be as independent as possible.
